New Patient Procedures:


  • Call us at 214-828-4702 to for a quick phone interview to obtain approval as a new patient. During this brief interview, the front office will ask about your insurance, discuss current medications/conditions, and obtain the source of referral.

  • Then we will schedule you for your new patient orientation session. The orientation session will include a tour of the facility, insurance verification, and record release procedures from the previous medical providers. If you have your previous medical records, you are encouraged to bring them with you to the orientation visit.

  • Download, print, and fill out the forms below. Bring the finished forms to your orientation session to expedite the check-in process.

  • Please do not email us the finished form as it will contain your personal health information and email is not a HIPAA secure communication tool for that type of data.
